This guide from the Connecticut State Department of Education/ Transition Task Force compares different tools, supports and federal requirements for serving students with disabilities or medical conditions. The guide separates the plans into the following core components:
• Legal references;
• Academic development;
• Career development; and,
• Social, emotional and physical development.

Across each core component the guide then differentiates between the needs of all students and the needs of students with disabilities or medical conditions within various plans. The needs for students with disabilities are further separated across the core components by the following types of plans: Individualized Education Plan (IEP), Summary of Performance (SOP), Section 504 plan, and Individualized Health Care Plan (IHCP).
